VFSC is thrilled you are joining in our Family Skate Night!
This is the liability waiver that VFSC requires each individual person to fill out and sign. Parent/Guardians may fill out the form on behalf of an individual under the age of 18.
Current VFSC skaters signed up for the 2023-2024 season does not need to fill out this form as it was part of registration.
Please note that you must fill out this form for each individual member skating on the ice. This form does not cover a full family.
If we do not show your name or any other name of your family skating - that individual will either need to sign a paper form or will be asked to wait until the form is signed.
**This form is good for the entire season of 2023-2024**
